Job Description:
As a Clinical Operations Director at CenterWell Home Health, you will oversee the coordination and delivery of high-quality direct care patient services in a home health setting. Your focus will be on ensuring quality assurance and operational excellence through effective planning, implementation, analysis, and organization of clinical operations for a specific location.
Key Responsibilities:
- Develop and implement clinical operations strategies to improve patient outcomes and care delivery
- Conduct thorough patient assessments and update care plans to meet individual needs
- Manage caregivers and oversee care delivery to ensure timely and effective services
- Monitor documentation compliance and ensure accurate final audits and billing
- Coordinate communication between team members and attending physicians to ensure seamless care planning and outcome management
Requirements:
- Graduate of an accredited School of Nursing with strong leadership experience
- At least two years as a Registered Nurse with one year of management experience in a home care or equivalent environment
- Proven track record in home health experience
- Preferred qualifications include Homecare Homebase (HCHB) experience and CMS PDGM billing knowledge
