Job Summary:
We are seeking a skilled Speech Language Pathologist to join our team. As a key member of our interdisciplinary care team, you will evaluate, direct, and provide speech/language pathology services to patients in the comfort of their own homes.
Duties and Responsibilities:
- Evaluate patient communication needs and develop individualized treatment plans to address these needs.
- Administer and interpret diagnostic tests and assessments to identify communication disorders and swallowing difficulties.
- Develop and implement effective therapeutic strategies to improve communication skills, including augmentative and alternative communication systems when necessary.
- Collaborate with patients, families, caregivers, and healthcare professionals to ensure seamless care transitions and optimal patient outcomes.
- Maintain accurate and detailed records of patient progress, assessments, and treatment plans.
- Participate in ongoing education and professional development to stay current with best practices in speech-language pathology.
Requirements:
- Certification of Clinical Competence in Speech-Language Pathology (CCC-SLP) granted by ASHA.
- Minimum six months' experience as a speech therapist/speech language pathologist.
- Home health experience preferred.
- Current and unrestricted state licensure.
- Current CPR certification.
- Good organizational and communication skills.
