Job Responsibilities:
- Plan and administer prescribed skilled physical therapy treatment and training for patients with varying levels of ability and need.
- Test/screen patients' physical strengths to inform treatment plans and adjust them as needed to achieve optimal outcomes.
- Develop and implement customized conditioning/rehabilitation programs tailored to each patient's unique needs and goals.
- Collaborate with physicians and clinical team members to ensure seamless care coordination and effective communication.
- Supervise and evaluate the performance of Physical Therapy Assistants and Home Health Aides, ensuring they meet high standards for quality and compassion.
Requirements:
- Master's degree in Physical Therapy from an accredited program
- Minimum of one year of physical therapy experience
- Current licensure as a Physical Therapist
- Current CPR certification
- Excellent communication and interpersonal skills
