About the Role
We are seeking a skilled Medical Social Worker to join our interdisciplinary care team at CenterWell Home Health.
In this role, you will evaluate and develop personalized care plans to address patients' emotional and social needs. You will collaborate with healthcare professionals to ensure effective communication and resource allocation, promoting optimal patient outcomes.
Your Key Responsibilities
- Evaluate patients' social and emotional states, assessing their needs for care and response to treatment.
- Assess relationships between patients' medical and nursing needs in the home setting, financial resources, and available community resources.
- Provide action to obtain available community resources to assist in resolving issues impeding patients' recovery.
- Teach patients and families in treating and coping with social and emotional responses connected with illness or injury.
Requirements:
- Masters or doctoral degree from a school of social work accredited by the Council on Social Work Education.
- Social worker licensure in the state of practice; if required by state law or regulation.
- A valid driver's license, auto insurance, and reliable transportation.
