Job Summary

Duties and Responsibilities:
- Assess patients' physical function and mobility to develop effective treatment plans.
- Provide one-on-one therapy sessions to patients in their homes, addressing specific needs and goals.
- Communicate regularly with patients' physicians and family members to ensure comprehensive care.
- Document all interactions, treatments, and patient progress accurately and efficiently.
Requirements:
- Doctoral degree in Physical Therapy (DPT) from an accredited institution.
- Active PT licensure in good standing.
- Minimum one year of clinical experience, preferably in home health or geriatrics.
- Current CPR/AED certification and ability to maintain a clean driving record.
