What You'll Do
- Assess patients' needs and develop individualized care plans
- Collaborate with other healthcare professionals to ensure comprehensive care
- Provide ongoing care and case management for each patient
- Confer with physicians in developing the initial plan of treatment
- Implement patient care plans in conjunction with patients and families
- Oversee the work of Home Health Aides, Certified Home Health Aides, and Personal Care Workers
Requirements
- Diploma, Associate, or Bachelor's Degree in Nursing
- OASIS proficiency
- Home Health experience required; HCHB experience preferred
- Current and unrestricted Registered Nurse licensure
- Current CPR certification
- Strong organizational and communication skills
