Our care teams include nurses, physical therapists, occupational therapists, speech-language pathologists, home health aides, and medical social workers – all working together to help patients rehabilitate, recover, and regain their independence.
We are seeking a skilled Medical Social Worker to join our team and contribute to this mission.
Key Responsibilities:
- Evaluate patients' social and emotional states, identifying areas where they may need support or intervention.
- Develop and implement care plans that incorporate community resources, patient education, and emotional support.
- Collaborate with healthcare professionals, including physicians, nurses, and therapists, to ensure comprehensive care.
- Provide guidance and supervision to Social Work Assistants as needed.
Requirements
- Masters or doctoral degree from a school of social work accredited by the Council on Social Work Education.
- Social Worker licensure in the state of practice (if required by state law or regulation).
- A valid driver's license, auto insurance, and reliable transportation.
- Proof of current CPR certification.
- Minimum of one year of experience as a social worker in a healthcare setting, home health, and/or hospice.
- Knowledge of and ability to assist with discharge planning needs and obtain community resources.
