Job Summary
As a Patient Care Specialist with CenterWell Home Health, you will play a vital role in providing personalized care to patients in their homes. We are seeking dedicated and compassionate individuals to join our team as Home Health Aides (HHAs).
In this role, you will provide direct patient care under the direction of a Registered Nurse (RN) and according to the Plan of Care (POC). You will assist patients with maintaining good personal hygiene, mobility, exercises, and positioning/turning, as per the POC and CenterWell Home Health policy. Additionally, you will communicate basic principles of nutrition, observe and record food and fluid intake when necessary, and report changes and document pertinent information and care rendered to ensure continuity of care.
