As a Patient Services Coordinator-LPN, you will play a vital role in our care teams that include nurses, physical therapists, occupational therapists, speech-language pathologists, home health aides, and medical social workers. **Responsibilities:**
- Schedule visits and communicate with field staff, patients, physicians, etc. to maintain proper care coordination and continuity of care.
- Assist with day-to-day office and staff management, including managing schedules for all patients.
- Initiate infection control forms as needed and send completed reports to the HRD for uploading in the worker console.
**Requirements:**
- Bachelor's degree in Nursing or related field
- At least 1 year of home health experience required
- Coding certification is preferred
- Excellent communication skills and ability to interact well with diverse group of individuals
