We are seeking a highly skilled and compassionate Physical Therapist to join our home health care team. The ideal candidate will have experience in home health care and possess excellent communication and interpersonal skills. In this role, you will work collaboratively with our multidisciplinary team to assess patient needs, develop individualized care plans, and provide evidence-based interventions to promote optimal recovery outcomes.
- Evaluate patient function and mobility, identifying areas for improvement and developing targeted interventions.
- Design and implement conditioning and rehabilitation programs tailored to each patient's specific needs and goals.
- Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination.
- Maintain accurate records of patient progress and treatment plans, including assessment data, treatment notes, and outcome measurements.
