As a Medical Social Worker, you will play a vital role in our interdisciplinary care team. Your expertise will enable you to assess and develop personalized plans of care tailored to each patient's emotional and social needs. You will work collaboratively with other healthcare professionals to provide comprehensive care that addresses the whole person - physically, emotionally, and socially.
Your primary responsibilities will include:
- Evaluating patients' social and emotional states as they relate to their illnesses or injuries.
- Assessing relationships between medical and nursing needs, financial resources, and available community resources.
- Providing action to obtain available community resources to resolve issues impeding patient recovery.
- Instructing patients and families on treating and coping with social and emotional responses connected to illness.
- Participating in the development and periodic re-evaluation of the physician's Plan of Care for the patient.
Required Skills and Qualifications
To succeed in this role, you must possess:
- A Master's or doctoral degree from a school of social work accredited by the Council on Social Work Education.
- Social worker licensure in the state of practice (if required by state law or regulation).
- A valid driver's license, auto insurance, and reliable transportation.
- Proof of current CPR certification.
- A minimum of one year of experience as a social worker in a healthcare setting, home health, and/or hospice.
