**Job Description**
We are seeking a skilled Speech Language Pathologist to join our team. As a key member of our care team, you will work with patients who are recovering from injury or illness, or managing chronic conditions. Your primary responsibilities will include evaluating, directing, and providing speech/language pathology services to patients in their homes or facilities.
**Key Responsibilities**
* Evaluate and assess patient's speech, language, voice, hearing, and swallowing disorders
* Develop and implement individualized treatment plans to address patient's communication and swallowing difficulties
* Provide instruction and training to patients and their families on use of alternative communication systems and strategies for improving communication and swallowing functions
* Participate in interdisciplinary care planning and coordination to ensure seamless transitions and optimal patient outcomes
**Requirements**
* Master's degree in Speech-Language Pathology or related field
* Current Certification of Clinical Competence in Speech-Language Pathology (CCC-SLP) granted by ASHA
* Minimum of six months experience as a speech therapist/speech-language pathologist
* Valid driver's license and reliable transportation
