Job Summary:
CenterWell Home Health seeks a skilled Occupational Therapist to provide high-quality care to patients in their homes. As an Occupational Therapist with our organization, you will be responsible for conducting evaluations, creating treatment plans, and implementing interventions to promote patients' independence and overall well-being.
Key Responsibilities:
- Evaluate patients' physical and cognitive abilities to identify areas for improvement.
- Develop individualized treatment plans to address patients' needs and goals.

Qualifications:
- Graduate degree in Occupational Therapy from an accredited program.
