As a physical therapist in home health, you will be responsible for planning and administering physical therapy treatments for patients with various injuries and functional disabilities. Key Responsibilities:
- Assess patient needs and develop individualized treatment plans in collaboration with medical teams.
- Plan and administer prescribed skilled physical therapy treatment and training for patients.
- Test/screen patients' physical strengths to assist physicians in evaluating their level of function.
- Develop and implement conditioning/rehabilitation programs consistent with physicians' Plans of Treatment.
- Confer with physicians and clinical team members to obtain additional patient information and assist in developing, implementing, and revising therapy treatment programs.
- Provide written instructions/care plans to Physical Therapy Assistants and Home Health Aide staff, supervise/evaluate their performance, and monitor the completion of documentation.
