Key Responsibilities:
- Evaluate, direct, and provide high-quality speech/language pathology services to patients in their homes or facilities.
- Participate in developing and regularly reviewing the Plan of Treatment and Plan of Care, ensuring patient-centered care.
- Utilize professional skills and judgment to assess and treat disorders of speech, voice, language, hearing, and swallowing, minimizing their effects.
- Administer and interpret diagnostic tests, apply therapeutic treatments, and observe changes in patients' conditions and responses to treatment.
- Counsel and instruct patients, families, and healthcare staff, promoting effective communication and collaboration.
- Prepare and submit timely written reports of evaluations, visits, summaries, care plans, care coordination activities, and progress reports, maintaining accurate documentation.
Required Qualifications:
- Meet the education and experience requirements for Certification of Clinical Competence in Speech-Language Pathology or Audiology granted by ASHA.
- Minimum six months of experience as a speech therapist/speech-language pathologist, with home health experience being beneficial.
- Current and unrestricted license, along with current CPR certification.
- Good organizational and communication skills, as well as a valid driver's license, auto insurance, and reliable transportation.
