About the Job
We're seeking a skilled Speech Language Pathologist to join our team. As a Home Health Speech Therapist, you will play a vital role in helping our patients rehabilitate, recover, and regain their independence.
As a member of our care team, you will work closely with nurses, physical therapists, occupational therapists, home health aides, and medical social workers to provide patient-centered care.
Responsibilities
- Evaluate, direct, and provide speech/language pathology services to patients in the home or facility
- Develop and implement individualized treatment plans to meet the unique needs of each patient
- Conduct diagnostic tests and assessments to identify communication disorders and develop effective treatment strategies
- Provide counseling and education to patients and their families on communication techniques and strategies
- Maintain accurate and detailed records of patient progress and treatment outcomes
Requirements
- Certification of Clinical Competence in Speech-Language Pathology or Audiology granted by ASHA
- Minimum of six months experience as a speech therapist/speech-language pathologist
- Home Health experience a plus
- Current and unrestricted license
- Current CPR certification
- Good organizational and communication skills
