About the Role:
">We are seeking a highly skilled and compassionate Speech Language Pathologist to join our team at CenterWell Home Health. As a key member of our care team, you will play a vital role in evaluating and providing speech and language services to patients in their homes or facilities.
">As a Speech Language Pathologist, your responsibilities will include:
">- Evaluating patient conditions and developing effective treatment plans to enhance communication abilities.
- Interpreting diagnostic tests and administering therapeutic treatments, including audio logic screening.
- Providing instruction and training to patients in the use of alternative communication systems when appropriate.
- Maintaining accurate documentation and submitting reports as required by Company policy.
- Participating in care coordination activities and discharge planning.
Requirements:
">- Meet the education and experience requirements for Certification of Clinical Competence in Speech Language Pathology or Audiology granted by ASHA.
- Minimum of six months experience as a speech therapist/speech language pathologist.
- Home Health experience is a plus.
- Current and unrestricted license.
- Current CPR certification.
- Good organizational and communication skills.
- A valid driver's license, auto insurance, and reliable transportation are required.
