Job Responsibilities
The successful candidate will be responsible for:
- Assessing and developing therapeutic programs for patients in their homes or other settings.
- Collaborating with the patient's physician and other healthcare team members to develop/revise the Plan of Care Treatment.
- Providing therapeutic treatment and instruction to patients in accordance with physician orders.
- Reporting patient status and progress to the physician, rehabilitation staff, Clinical Manager, patient, and family members.
- Participating in care coordination and discharge planning activities.
