About the Role
CenterWell Home Health is seeking a highly skilled Occupational Therapist to join our team. As an Occupational Therapist, you will assess and develop rehabilitation programs tailored to each patient's needs to improve their daily living skills.
Key responsibilities include assessing/screening patients' daily living/work-related skills and developing therapeutic retraining programs with measurable objectives. You will also confer with the patient's physician and other healthcare team members to develop/review the Plan of Care Treatment.
In addition, you will provide therapeutic treatment and instruction to patients in accordance with physician orders to improve/restore strength, coordination, range-of-motion, and function or teach compensation measures.
- Develop personalized rehabilitation plans for patients.
- Collaborate with healthcare teams to ensure effective care.
- Provide therapeutic support and instruction to patients.
Requirements
- Master's degree from an accredited Occupational Therapy Program.
- At least six months of occupational therapy experience preferred.
- Current and unrestricted OT licensure.
- Current CPR certification.
- Good organizational and communication skills.
