Key Responsibilities
- Evaluate patients' daily living skills and develop comprehensive treatment plans to address functional deficits
- Conduct thorough assessments to identify areas for improvement and create targeted interventions
- Collaborate with healthcare teams to ensure cohesive care delivery and optimal patient outcomes
- Provide direct patient care, including education on proper techniques and adaptive strategies for daily activities
- Document patient progress and communicate findings to healthcare teams as necessary
- Participate in quality improvement initiatives to advance best practices and enhance patient satisfaction
Requirements
- Bachelor's degree in Occupational Therapy from an accredited program
- Current licensure as an Occupational Therapist (OT) in good standing
- Minimum 1 year of experience in home health or similar setting
- Excellent communication and interpersonal skills, with ability to work effectively with diverse populations
- Strong analytical and problem-solving abilities, with capacity to adapt to changing situations
- Valid driver's license and reliable transportation for travel to patients' homes
