Job Description:
A caring and compassionate Home Health LPN is needed to join our team. As a key member of our care team, you will provide professional nursing services under the direction of a Registered Nurse (RN) in compliance with applicable laws and regulations, as well as CenterWell Home Health policies and procedures.
Key Responsibilities:
- Provide skilled nursing interventions in the treatment of patients' illnesses, rehabilitative needs, and preventative care.
- Utilize a holistic approach in providing problem-specific therapeutic interventions, teaching, and training activities in accordance with the plan of care.
- Maintain contact with patients, physicians, clinical managers, and other healthcare team members in a timely manner regarding patient/family needs and status changes.
- Participate in care coordination activities and discharge planning as appropriate.
Requirements:
- Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing.
- Current nursing license in the practicing state.
- Two years of experience as an LPN/LVN in a clinical setting, preferably in a home health or hospice setting.
