Medical Social Worker
Participate in interdisciplinary care provided to home health patients, evaluating and developing personalized plans of care to meet emotional and social needs.
Key Responsibilities:
- Evaluate patient's social and emotional state related to illness or injury, needs for care, and response to treatment.
- Assess relationships between medical and nursing needs, financial resources, and available community resources.
- Provide action to obtain community resources resolving issues impeding recovery.
- Instruct patients and families in treating and coping with social and emotional responses.
- Provide ongoing assessment of patient and family needs and responses to teaching.
- Assist physician and other health team members understanding significant social and emotional factors related to patient's health.
Requirements:
- Masters or doctoral degree from a school accredited by the Council on Social Work Education.
- Minimum one year experience as a social worker in a healthcare setting, home health, and/or hospice.
- Knowledge of discharge planning needs and ability to obtain community resources.
- Fluent English reading, writing, and speaking skills.
- Ability to be flexible in work hours and travel locally.
