About the Role:
We are seeking a compassionate and skilled professional to join our care team as a Home Health Occupational Therapist. As a valued member of our team, you will play a crucial role in helping patients recover from illness, injury, or surgery.
Key Responsibilities:
- Develop and implement patient care plans in collaboration with physicians and healthcare professionals.
- Design personalized retraining programs tailored to each patient's unique needs and goals.
- Provide regular updates on patient progress and status to healthcare staff, family members, and caregivers.
- Cross-functional coordination and guidance to ensure seamless care delivery.
- Develop community-based rehabilitation activities to aid patients in physical recovery and psychological adjustment.
- Recommend adaptive equipment to enhance patients' living and working environments.
- Train staff and family members on occupational therapy techniques and strategies.
- Evaluate and oversee the performance of occupational therapy assistants and home health aides.
Requirements:
- A Master's degree in Occupational Therapy from an accredited program.
- At least six months of experience in occupational therapy (home health experience preferred).
