Job Summary
We are seeking a dedicated and compassionate Occupational Therapist to join our CenterWell Home Health team. As a key member of our interdisciplinary team, you will work closely with patients, families, and healthcare professionals to promote optimal health outcomes.
Your primary responsibility will be to assess and develop individualized rehabilitation programs that address the unique needs of each patient. This may involve administering and interpreting diagnostic and prognostic tests of function, conferring with healthcare team members, and providing therapeutic treatment and instruction.
Additionally, you will participate in care coordination and discharge planning activities, ensuring a seamless transition of care for patients. Your expertise and passion for occupational therapy will make a meaningful difference in the lives of our patients and their families.
- Assess patients' functional abilities and develop targeted interventions.

Qualifications
- Bachelor's degree from an accredited Occupational Therapy Program.
- Minimum of six months of occupational therapy experience.
- Current and unrestricted OT licensure.
- Current CPR certification.
