About the Role
As a Personalized Home Health Services Coordinator at CenterWell Home Health, you will be responsible for:- Providing skilled nursing visits and case management services to patients in their homes.
- Developing and implementing individualized treatment plans tailored to each patient's unique needs and goals.
- Collaborating with physicians, therapists, and other healthcare professionals to ensure effective coordination of care.
