Job Title: Home Health Physical Therapist

Key Responsibilities:
- Develop and implement customized physical therapy treatment plans for patients with various injuries, illnesses, and functional disabilities to maximize their physical function.
- Assess patients' physical strengths to inform physicians about their level of function and record findings to create effective treatment programs and measurable objectives.
- Design and implement rehabilitation programs aligned with physician-developed plans of treatment and the overall goals of the patient/rehabilitation team.
- Collaborate with physicians and clinical team members to gather additional patient information and contribute to developing, implementing, and revising therapy treatment programs and plans of treatment.
- Supervise and evaluate the performance of Physical Therapy Assistants and Home Health Aide staff, providing written instructions and care plans that reflect current treatment strategies.
- Monitor the completion of documentation by physical therapy assistants and home health aides/personal care workers as part of supervisory/leadership responsibilities.
- Accurately document patients' care observations, interventions, and evaluations in a timely manner.
- Ensure the implementation of interim orders received from physicians and physical therapists, documenting and submitting them for physician signature.
- Communicate patients' progress to their physicians, Clinical Manager, staff, patient, and family members.
Required Qualifications:
- Degree from an accredited Physical Therapy Program (approved by the American Physical Therapy Association).
- Minimum one year of physical therapy experience preferred.
- Current and unrestricted Physical Therapy license.
- Current CPR certification.
- Strong organizational and communication skills.
