We are seeking a skilled Physical Therapist to join our team. As a Physical Therapist at CenterWell Home Health, you will work with patients to develop personalized rehabilitation plans, provide physical therapy treatments, and conduct assessments to monitor patient progress.
Your responsibilities will include:
- Developing and implementing individualized rehabilitation plans for patients
- Providing physical therapy treatments and interventions to improve patient mobility and function

Requirements
To be successful in this role, you must have:
- A degree from an accredited Physical Therapy program (approved by the APTA)
- Minimum of one year of experience as a Physical Therapist
- Current and unrestricted Physical Therapy license
- Current CPR certification
