Roles and Responsibility
- Provide comprehensive nursing care to patients in their homes, including patient assessment and monitoring vital signs.
- Administer medications and treatments, such as IV insertion and tube feeding, while ensuring adherence to safety protocols.
- Utilize electronic health record (HCHB) systems for accurate documentation and communication of patient information.
- Collaborate with interdisciplinary teams to develop and implement individualized care plans.
- Educate patients and families on health management and provide support for daily living activities.
Skills
Homecare Homebase Software Experience - Required
OASIS Experience - Required
Current state license as a Registered Nurse - Required
CHPN certification - Preferred
Proof of current CPR certification - Required
Current driver's license - Required
Must have reliable transportation and insurance - Required
1-2 years experience as a Registered Nurse in a clinical care setting - Required
2 years Home health experience - Required.
Education
Graduate of an accredited Associate Degree in Nursing or Nursing Diploma program - Required
Bachelors of Science in Nursing - Preferred

Pay: $2,200.00 - $2,300.00 per week
