As a Home Health Registered Nurse at CenterWell Home Health, you will have the opportunity to work with a diverse range of patients and their families, providing personalized care and support to help them achieve optimal health outcomes. Your responsibilities will include assessing patients' needs, developing and implementing individualized care plans, and collaborating with other healthcare professionals to ensure comprehensive care. Key Responsibilities
- Provide skilled nursing care to patients in the comfort of their own homes
- Develop and implement individualized care plans in collaboration with patients, families, and other healthcare professionals
- Collaborate with physicians to ensure comprehensive care and optimal health outcomes
- Supervise the work of Home Health Aides, Certified Home Health Aides, and Personal Care Workers
- Monitor the appropriate completion of documentation by home health aides/personal care workers
Requirements
- Diploma, Associate, or Bachelor's Degree in Nursing
- OASIS proficiency
- Home Health experience required; HCHB experience preferred
- Current and unrestricted Registered Nurse licensure
- Current CPR certification
- Strong organizational and communication skills
