About the Position
We are currently seeking a highly motivated and detail-oriented Physical Therapist to join our team as a Skilled Home Health Therapy Specialist.
Responsibilities:
- Provide comprehensive physical therapy services to patients in their homes
- Develop and implement individualized treatment plans to address patients' unique needs and goals
- Collaborate with interdisciplinary teams to ensure seamless care coordination
- Communicate effectively with patients, families, and caregivers to promote informed decision-making and empowerment
- Maintain accurate and timely documentation of patient progress and treatment outcomes
Qualifications:
- Degree from an accredited Physical Therapy program
- Current licensure as a Physical Therapist
- Minimum one year of experience in home health or related field
- Excellent communication and interpersonal skills
