As a Home Health Speech Language Pathologist, you will:
- Evaluate, direct and provide speech/language pathology services to patients in the home or facility setting.
- Contribute to the development and periodic review of patient treatment plans and care coordination activities.
- Utilize your professional expertise to assess and treat disorders of speech, voice, language, hearing, and swallowing, with a focus on preventing, identifying, evaluating, and minimizing their effects.
- Administer and interpret diagnostic tests and therapeutic applications, including audiologic screenings.
- Monitor patient progress, recording and reporting changes in condition and response to treatment to supervisors and physicians.
- Provide education and training to patients on alternative communication systems when appropriate.
- Counsel and instruct patients, families, and healthcare staff on various aspects of speech-language pathology.
- Maintain accurate documentation and submit timely written reports as required by company policy.
- Participate in care coordination activities, discharge planning, and other related efforts.
- Adhere to strict confidentiality standards, sharing information only when the recipient's right to access is established and in the best interests of the patient.
- Pursue ongoing education and training to enhance clinical competencies and stay current with industry developments.
Required Qualifications:
- Meet ASHA certification requirements for Clinical Competence in Speech-Language Pathology or Audiology.
- Hold a minimum of six months' experience as a speech therapist/speech-language pathologist.
- Prior home health experience is highly valued.
- Current and unrestricted licensure.
- Current CPR certification.
- Excellent organizational and communication skills.
- A valid driver's license, auto insurance, and reliable transportation are essential.
