Requirements:
- Degree from an accredited Physical Therapy Program
- Minimum one year of physical therapy experience preferred
- Current and unrestricted Physical Therapy license
- Current CPR certification
- Strong organizational and communication skills

Job Summary:
We are seeking a skilled and compassionate Home Health Physical Therapist to join our team. The successful candidate will be responsible for delivering personalized physical therapy treatments to patients in their homes, helping them regain their independence and improve their overall health. Key responsibilities include:
- Provide skilled physical therapy treatment and training to patients with various injuries or functional disabilities
- Develop and implement conditioning and rehabilitation programs consistent with physician-approved plans of treatment
- Collaborate with healthcare teams to develop and revise therapy treatment plans as needed
- Supervise and evaluate the performance of Physical Therapy Assistants and Home Health Aides
Requirements:
