CareWell Home Health is seeking a skilled Occupational Therapist to join our team. As an Occupational Therapist, you will play a vital role in helping patients recover from injury or illness and regain their independence.

Responsibilities:
- Confer with the patient's physician and other healthcare team members to develop and revise the Plan of Care Treatment.
- Review and expand retraining programs in accordance with each patient's behavioral goals and the rehabilitation program's guidelines.
- Report patient status and progress to the physician, rehabilitation staff, Clinical Manager, patient, and family members.
- Participate in care coordination and discharge planning activities and serve as a resource to other healthcare personnel in meeting patients' needs.
- Design community reintegration activities to assist clients in physical reconditioning efforts and psychological adjustment, coordinating plans with the interdisciplinary team.
- Recommend and design special adaptive equipment for clients to improve residence or working environments or enhance participation in the rehabilitation program and/or community.
- Train staff and family members to carry out retraining programs, including dressing, feeding, grooming, and hygiene skills, participating in team and family educational meetings.
- Coordinate, oversee, supervise, instruct, and evaluate Occupational Therapy Assistant and Home Health Aide performance in implementing occupational therapy services.
- Accurately document patients' care observations, interventions, and evaluations on the day services are rendered. Ensure that interim (verbal) orders received from the physician are accurately documented and implemented.
- Submit evaluation, treatment plans, and discharge summaries to the supervisor and care management staff.
