Key Responsibilities:
- Provide proactive, preventive care to seniors through wellness visits, physical exams, chronic condition management, and screenings.
- Collaborate with other healthcare professionals to develop personalized care plans that address the unique needs of each patient.
- Prescribe medication, examine patients, diagnose illnesses, and provide treatment in accordance with established medical guidelines.

Requirements:
- Bachelor's degree in a relevant field, such as nursing or medicine.
- 8 years of technical experience in a clinical setting.
- 2 years of project leadership experience.
- Active, unrestricted Nurse Practitioner/PA license in the appropriate state.
