Main Responsibilities:
- Oversee the delivery of high-quality care to seniors, ensuring that all patients receive the best possible treatment.
- Evaluate and treat patients in accordance with standards of care.
- Coordinate care services, including but not limited to specialty referrals, hospital and SNF coordination, durable medical equipment and home health care.
Requirements:
- Board Certification or Eligible to become certified (ABMS or AOA) in Family Medicine, Internal Medicine or Geriatric Medicine preferred.
- At least 2 years of relevant experience.
