Physical Therapy Per Diem.
8am-4 : 30pm.
Start Date : ASAP START.
Requires recent in-patient acute experience.
Agency employee shall report to the staffing office located in the Main hospital 2nd floor.
Dress professional any color, parking lot number 6 (validation available).
Job Requirements
Valid for 2 years. Available for free at https : / / calcivilrights.ca.gov / shpt / Covered in WAMC orientation packet when they come into facility under Essentials.
Primary Source Verification of professional license, and / or certification, as required for the position. Ran within 30 days of start. Updated upon expiration.

As of 1 / 01 / 24 California law permits the testing for marijuana psychoactive metabolites only by saliva. All drug screens will be required to have two components for onboarding at all CA Prime facilities.
A 10-panel urine drug screen excluding THC and a psychoactive THC saliva test.
Relias accepted. Must be completed annually.
Completed after all other credentials are uploaded and with most up to date information.
National Criminal Background Check within one year of start date.
Additional Details
For Covid 19 vaccine is required annually, declination is accepted needs to show for 2024-2025.
Proof of 2 Vaccines or Positive Titer showing immunity. Declinations accepted for this facility only.
