Job Summary:

Responsibilities:
- Provide physical therapy services to patients in a SNF / LTC setting
- Develop and implement individualized treatment plans to meet patient needs
- Conduct assessments and evaluations to determine patient progress
- Collaborate with healthcare team members to ensure comprehensive patient care
Requirements:
- 1 year of recent experience as a Physical Therapist in a SNF / LTC setting
- Current Physical Therapist license in the state of Tennessee
