About the Position
This is a 5x8 days contract position, requiring the successful candidate to work a variety of shifts, including 8 hours per day, with a guaranteed minimum of 40 hours per week. As a contract Physical Therapist, you will be responsible for providing high-quality patient care, developing and implementing individualized treatment plans, and collaborating with interdisciplinary teams to achieve optimal patient outcomes.
Key Responsibilities
- Develop and implement individualized treatment plans to meet the unique needs of each patient
- Conduct thorough assessments to identify patient goals and priorities
- Provide evidence-based interventions to achieve optimal patient outcomes

Requirements
- Current Physical Therapist license within the state of practice
- 1 year of recent experience as a Physical Therapist specializing in SNF/LTC
- Strong communication and interpersonal skills
