In this role, you will be responsible for assessing patients' needs, developing treatment plans, and implementing evidence-based interventions to promote optimal outcomes. You will work closely with multidisciplinary teams to provide exceptional care to patients in SNF/LTC settings.
Requirements
To succeed in this role, you should possess excellent communication and interpersonal skills, with the ability to work effectively with multidisciplinary teams. Strong clinical skills, including assessment, diagnosis, and treatment planning, are essential. A current PT license in the state of California and at least 1 year of recent experience in SNF/LTC are also required.
