What are the responsibilities and job description for the Case Manager Physical Therapy (Full Time, Lynchburg Home Health) position at Centra Medical Group?
The Home Health Physical Therapy Case Manager evaluates, develops a plan of care and treats patients to maintain or restore patients to their maximum functional performance level and completes discharge planning. Provides supervision for Licensed Physical Therapist Assistants and Certified Nursing Assistants.
Required Education: Bachelors Degree in Physical Therapy
Preferred Education: Masters Degree in Physical Therapy, or Doctoral Degree in Physical Therapy
Required Experience: For Bachelors Degree, five (5) years of experience as a Physical Therapist
Preferred Experience: Skilled Nursing Facility experience
Required Certifications and Licensures: Licensed by the Virginia Board of Physical Therapy
Implements action plans to achieve or exceed service excellence benchmarks
Evaluates and develops plan of care according to American Physical Therapy Association (APTA) guidelines
Administers physical therapy treatments appropriate to diagnosis and to APTA guidelines
Appropriately documents and synchronizes patient care according to federal, state, and local regulations
Meets productivity standards
Uses resources (e.g., time, people, supplies, etc.) cost effectively
Supervises Licensed Physical Therapy Assistants, Certified Nursing Assistants and students
Protects patients, employees, and visitors by promoting infection-control and safety policies and protocols
Plans, coordinates, manages, and documents the care provided to assigned patients
Ensures appropriate patient and/or caregiver education with utilization of applicable resources
Leads, communicates and collaborates with the interdisciplinary team to plan patient care
Adheres to National Patient Safety Goals (NPSG)
Participates in performance improvement initiatives affecting patient outcomes
Handles conflict resolution in an appropriate, timely, and consistent manner
Identifies personal contributions to department cascading goals
Practices within physical therapy licensure guidelines
Provides one in-service annually to staff
Engages in own professional development as evidenced by self assessment and attainment of degrees, certifications, and continuing education
Contributes to providing a supportive environment that is conducive to learning for all students from all academic programs
