What are the responsibilities and job description for the SPECIALIST OUTCOMES- Lynchburg General Hospital- Full-Time/Days position at Centra Medical Group?
The Outcomes Specialist plans, organizes, and implements patient safety and clinical quality process improvements. The Outcomes Specialist will assist with data collection through chart reviews and other methods; work with members of Performance Improvement Nurses Physicians and ancillary departments to identify opportunities for patient safety clinical outcomes and practice variation; and assist with driving improvements.
Required Education: High school diploma or equivalent.
Preferred education: BSN.
Required Experience: 1 year of clinical experience. Strong written and verbal communication skills; ability to communicate with and influence leadership teams. Travel to Centra locations throughout central and south Virginia is required.
Required Certification & Licensures: Valid Virginia driver license.
Preferred Certification & Licensures: Hold a current, active license as a practical nurse in Virginia or hold a current multistate licensure privilege as a practical nurse.
Assists with quality improvement education for Centra staff and Medical staff.
Attends and presents accurate timely and relevant reports at department and interdisciplinary team meetings.
Attends meetings committees and conferences required of the position and to advance knowledge and experience.
Facilitates improvement activities to promote efficient and effective processes of care to yield optimal clinical outcomes.
Identifies and trends significant outcomes and performance data. Identifies opportunities to enhance clinical outcomes.
Organizes and leads diverse clinical and organizational teams to focus on efficiency quality indicators and patient needs.
Reliable collection of clinical data through effective utilization of the medical record and information systems.
Reports and provides clear guidance to staff regarding clinical quality measures and processes.
Serves as an educational resource for internal and external audiences by developing educational material and delivering presentations related to patient safety and clinical outcomes. Effectively coaches staff in one-on-one situations.
