Job Summary and Scope
As a member of the Clinical Services team, works primarily in local jail facilities to conduct screenings, assessments, jail diversion and crisis activities. Staff must possess computer, typing, grammar, punctuation, proofreading, transcription, phone proficiencies and excellent communication and customer relations skills to effectively interact with staff, customers, business partners and community stakeholders. Additionally, the employee must exercise independent decision-making and work effectively in a team-based environment. The employee will complete other tasks as needed in various programs as assigned by supervisor.

Position Specific – Duties and Responsibilities
Job Specific :
- Establishes and maintains a liaison relationship with the jails in Manistee and Benzie counties. Maintains a consistent schedule of visitation to both county jails in order to facilitate the timely referral of inmates for mental health services.
- Provides clinical mental health assessments, SUD assessments, groups and referrals designed to stabilize, improve functioning and help individuals to obtain and maintain recovery from severe mental illness and co-occurring disorders as appropriate.
- Coordinates, links and monitors community resources, legal, mental and physical health services to assist the inmates in obtaining stability. Assists inmates in accessing entitlements, housing, employment, and other supports. Provides assessments, planning, linking, coordinating, monitoring and evaluating progress towards goals, and advocacy through community-based care.
- Provides ongoing training for corrections officers and law enforcement staff at community law enforcement and jail sites pursuant to the affiliation wide jail diversion practice guidelines in basic concepts related to mental illness and substance use, pre-booking diversions.
- Provides timely clinical documentation in the electronic medical record that includes amount, scope and duration of services provided in accordance with the person-centered plan. Complies with agency policy, DCH, CARF, and Medicaid criteria for clinical documentation for all aspects of the medical record including, but not limited to, intake assessments, person-centered plans, plan reviews, releases of information, progress / activity notes, annual psycho-social assessments and discharge summaries.
- Maintains ongoing cooperative working relationships with local agencies, community hospitals, employment agencies, regional institutional staff and appropriate state agencies; coordinates delivery of services with other community agencies.
- Participates in meetings and trainings as requested to maintain or improve proficiency.
- Contributes to team effort by accomplishing related results as needed.
- Complies with all policies and procedures of the agency.

Qualifications, Skills, and Conditions
Knowledge, Education and Training (licenses, programs, or certificates) :
Experience : A minimum of one year of progressively more responsible or expansive experience within the mental health field is required. Experience in substance abuse assessment and treatment is preferred. Lived experiences with mental illness, developmental disabilities, and / or substance use disorders valued.
Interpersonal and Communication Skills : Regularly requires a high level of interpersonal and communication skills such as resolving disagreements, disputes, or conflicts; leading training and educational sessions, making presentations, and applying formal counseling skills on a basic level.
Guidance / Work Direction Received : Vast majority of job duties are performed in accordance with well-defined policies, procedures and instruction, the incumbent uses some discretion in applying policies and procedures prescribed by management.
Supervisory or Managerial Responsibility : No supervisory or managerial authority.
Visual Concentration : Requires an average level of visual concentration on a regular basis.
Job Complexity : Requires planning, solving problems, analyzing situations, making decisions, or thinking creatively. The level of complexity involved may include inferential elements at times.
