Join the CentraCare team as a Care Coordinator Registered Nurse at the Pediatrics Clinic located at the Plaza.
The Pediatric Care Coordinator RN will function autonomously and in collaboration with multiple healthcare providers as a patient advocate, educator, and consultant for the designated pediatric department population. The position will focus on achieving quality clinical outcomes by effectively coordinating care for patients throughout the disease continuum and across multiple settings by utilizing resources effectively and efficiently. The pediatric care coordinator will apply the process of assessment, planning, implementation, monitoring, and evaluation of coordinated care efforts. The coordinator will maintain professional competencies by attending educational programs, participating in professional organizations, or other appropriate activities to serve as an expert resource to patients and families.
Schedule:
- Full Time | 72 hours per 2-week pay period
- 7:45am-5:15pm Monday, Wednesday, Thursday, and Friday
- Tuesdays off
- No evenings, weekends, or holidays!
Pay and Benefits:
- Starting pay is $31.86 per hour; exact wage determined by years of related experience.
- Pay range $31.86- $47.81

Requirements:
- Minimum of an associate degree of an accredited school of nursing.
- Current licensure as a registered nurse in the State of Minnesota.
- Current American Heart Association (AHA) BLS Healthcare Provider card required per established policy.
- Possesses the ability to understand patients’ medical records, physician orders and medication orders to be communicated to the patient and clinic staff.
- Strong organization skills and ability to work independently and in a team setting with minimal supervision.
- Ability to maintain customer service standards.
Preferred Qualifications:
- Experience with the diversity of the local community.
- Previous experience with care coordination.
- Previous experience in Pediatrics.
