Intake RN | Home Health/Hospice

Find your purpose with CentraCare working as an Intake Registered Nurse in Home Health/Hospice! In this role, you will be responsible for the assessment of patient needs, eligibility for services and start of care priority for all patients referred for home care or hospice services. This role functions as a clinical resource for staff, enhancing professional and independent practice within the department. 

Additional key responsibilities:  

• Ensure a seamless transition from referral to admission for patients and families.
• Identify payer sources and coordinate all eligibility and authorization requirements to secure payment for services delivered.
• Secure initial authorization and coordinate ongoing authorizations.
• Collaborate with the multidisciplinary team through Relationship-Based Care to support a healthy work environment and ensure a culture of safety.

Schedule

• Full-time | 64 hours every two weeks | Waite Park Home Health/Hospice Office  
• Monday, Tuesday, Thursday, Friday and every 4^th weekend | 8:00 am – 4:30 pm 

Pay and Benefits

• Starting pay begins at $37.27 per hour; exact wage determined by years of related experience 
  • Pay range: $37.27 - $55.91 per hour

Qualifications

• Bachelor's degree or higher degree in nursing required
• Current Minnesota Registered Nurse license 
• Minimum of one year of home care experience and knowledge of home care regulatory guidelines

Based out of our office in in Waite Park at the Marketplace Mall, the Home Health department primarily takes place in the communities we serve and in the homes of our patients.
