Find your passion for Social Work as a Social Worker II at the St. Cloud Hospital. This Part-time position will have the primary responsibility to support the patients in our 40-bed Level II Emergency Trauma Center.
At the St. Cloud Hospital, our Socials Workers serve patients and families in our community and beyond by helping them during a challenging time in their lives. Our Social Workers enjoy the variety of cases they are presented with as there is always something new and different. You will be able to collaborate with a large team of other Social Workers to gain knowledge and experience while also working with an interdisciplinary team made up of nurses, therapists, physicians, and dieticians. You will have a designated patient caseload and will plan care to effectively meet the patient/family transitional needs including monitoring the length of stay and promoting community resources. This position will be a part of the Care Management department. This role will work with an Interdisciplinary team within the Emergency Trauma Center. You will work with a vast population from infant to adult with a variety of medical and psychosocial needs. The ideal candidate will be a strong individual that is able to quickly assess and recognize the barriers and interventions needed and proceed independently as well as with a team.
We will provide supervision for new grads in-house or will reimburse up to $175/month for external supervision.

- Starting pay begins at $31.10 per hour and increases with experience
- Pay range $31.10 - $46.66 per hour

- Qualifications
- Master's Degree of Social Work required.
- Familiar with laws regarding confidentiality, vulnerable adult, and patient’s rights.
- Licensed Graduate Social Worker (LGSW) In State of Minnesota.
- American Case Management Association ACMA Case Manager-SW Certification or obtain among hire within 5 years.
- 1 year or more of clinical experience as a social worker preferred.
Hiring bonus requirements: To be eligible for the hiring-on bonus you must be an external candidate who is hired into an eligible full time or part time benefited position. Previous employees qualify if they have not worked for CentraCare for at least 9 months at the date of hire.
Relocation assistance requirements: Must accept a position that offers benefits and may not have worked for CentraCare in the last 9 months. Must currently reside more than 50 miles away from St. Cloud and relocate to within 30 miles of it.
