Experienced Social Worker Needed for Busy ADHC
Overall Objective : Provide social services to assist ADHC participants and their families to solve medical-social problems that arise as a result of the participant’s disability.
Qualifications :
A Bachelor’s (or master’s) degree from an accredited institution in Social Work
Demonstrated competence of the physical, mental and social needs of the elderly, and the ability to work in that environment.
Responsibilities :
The Social Worker is directly accountable to the Program Director for the achievement of the position’s responsibilities.
Be a member of the Multidisciplinary Team to evaluate medical-social needs of all participants, to assist participants and their families to solve medical-social problems that arise as a result of the participant’s medical condition or disability.
Provide initial and continuing assessment of each participant in the formation of an individual plan of care by the Multidisciplinary Team.
Develop a care plan if indicated, including short and long-term goals with each participant’s family and other appropriate individuals.
Serve as a liaison with the participant’s home and family; and with other community agencies that may be providing services to a participant; coordinate all services delivered to the participant to meet the participant’s needs and avoid duplication .
Duties :
Interview and screen all referrals to determine the general appropriateness of each prospective participant for the full assessment process and adult day care participation.
Provide a referral for persons not appropriate for adult day health care.
Provide a signed and dated documentation for all service performed the day the service is provided and include signed and dated quarterly progress reports in each participant record.
Provide for periodic reevaluation and revision of plan of care.
Maintain signed progress records in the participant’s health record at least quarterly.
Provide in-home assessment for all participants.
Provide group sessions in the program : promote peer group relationships through problem-centered discussion and task-centered groups.
Provide participant, caregiver and family counseling as needed.
Provide counseling and referral to other community resources.
