POSITION SUMMARY
The OCP Supervisor plays a key role in managing the day-to-day operations of the OCP program and overseeing the Long-Term Support Coordinators (LTSCs). The supervisor is responsible for ensuring that both the contractual requirements of the OCP and the standards set by the CBES are met. This includes managing LTSCs, supporting their professional development, ensuring adherence to policies and procedures, and acting as a liaison with various teams and external organizations.
ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:
- Lead weekly supervision meetings with LTSCs, providing guidance and support to the team.
- Conduct performance evaluations and manage other administrative tasks like approving timesheets and scheduling time off.
- Provide consultation to LTSCs regarding assessments, service planning, resource utilization, time management, and problem-solving.
- Accompany LTSCs on home visits to provide hands-on training, evaluations, and support.
- Assist LTSCs in their professional growth and development of career-related goals.
- Ensure that OCP and CBES policies and procedures are consistently followed by staff.
- Serve as a liaison between One Care Plans, CBES staff, community agencies, and consumers.
- Review and update monthly member rosters, assigning consumers to LTSCs based on language needs and caseload balance.
- Assist in resolving billing discrepancies and other administrative tasks.
- Participate in internal and external meetings, including CBES staff meetings and agency committees, sharing information and contributing to tasks.
- Attend and participate in relevant training, both internally and externally, to enhance role-specific skills and knowledge. Share learning outcomes with staff.
- Participate in hiring and orienting new LTSCs.
- Organize and update resource information, program forms, and related materials.
- Maintain a caseload of 20-30 consumers.
- Perform other tasks as needed and assigned by management.
EDUCATION AND EXPERIENCE:
- Bachelor’s degree preferably in social work, human services, nursing, psychology, or sociology, or a minimum of three years experience in a human service-related field, preferably working with elders.
- Two years working in an Aging Services Access Point (ASAP) organization with case management experience, with at least one year of supervisory experience strongly preferred.
- Experience working with SCO, One Care or other Managed Care Plans and knowledge of their database systems preferred.

PHYSICAL REQUIREMENTS OF THE JOB:
- Ability to lift and carry objects frequently weighing up to 10 to 15 pounds.
- Ability to climb stairs.
- Ability to travel to the community to perform visits at consumers’ homes.
- Work requires regular standing, stooping, and bending.
