Location: Central City Concern - Multiple Sites
*Note: This position will, indeed, be "floating" across multiple CCC sites. The Float Nurse (RN) is eligible to cover for primary care clinic nurse shifts at Blackburn (East Portland); Old Town Clinic and OTRC (Downtown); Hansen, LOC, and Evergreen Crossing (Northeast).
Schedule: Monday - Friday (7:45am - 5:15pm) 1/2 Day (Half day to fall on either Tuesday or Thursday)
Compensation: $44.05 - $57.27/Hour
SUMMARY: Nurses in this position will be eligible to cover for primary care clinic nurse shifts at Blackburn, Old Town Clinic, OTRC, Hansen, LOC, and Evergreen Crossing. The clinic nurse provides comprehensive community and clinic health services directed towards the prevention of disease, the promotion of health, and the client's self-care using professional judgment in comprehensive assessment, screening, nursing diagnosis, care planning, counseling, intervention, case management, health information /education, referral and evaluation.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Provide direct care and delegate care of patients as indicated by patient needs.
- Administer medications and treatments as directed by the standing orders and/or by physician. Utilize Pyxis and maintain electronic bed list.
- Observe and record pertinent information with regard to the care and condition of patients.
- Formulate individualized nursing care plans and participate in patient care conferences.
- Make referrals to hospitals and other agencies.
- Provide patient care and other work related directives for Subacute Technicians.
- Assist the staff physician. Respond in emergency situations.
- Give and receive report at shift changes.
- Consult with other health team members to lend guidance as necessary.
- Work closely with clinical staff ensuring integrated patient care.
- Adhere to all state and federal privacy regulations, including HIPAA and 42 CFR Part 2, and to CCC policies and agreements regarding confidentiality, privacy, and security. Support compliance with all privacy and security requirements pursuant to community partners' and outside providers' patient confidentiality agreements, including privacy and security requirements for EMR access. This includes immediately reporting any breach of protected health information or personal identification information of any person receiving CCC services by CCC or an outside provider to the CCC Compliance Department, as well as to your supervisor or their designee.

SKILLS AND ABILITIES:

- Ability to maintain accurate records and necessary paperwork.
- Ability to learn and apply training instruction.
- Knowledge of medical assessment with intoxicated persons.
MINIMUM QUALIFICATIONS:
- Successful completion of a course of accredited study that leads to the AD, or BS or AA degree in Nursing from OSBN or State Board approved course.
- Current RN license in the State of Oregon.
- BSN/QMHP preferred.
- Must meet CCC privileging requirements as required by FTCA.
- Must understand the causes of homelessness and poverty, the medical and psychosocial sequelae of homelessness and the systems and processes used to help complex and medically vulnerable participants secure and maintain stable housing, healthcare, and other social services.
- Six months of nursing experience and/or recent educational preparation working with either psychiatric or chemical dependency populations. One-year experience with either population preferred.
- Must have experience working with high-risk participants.
- BLS certification including child/infant certification is required prior to start date.
- Must generally have the ability to walk, bend, stoop, kneel, squat, twist, reach, pull and lift heavy objects, and climb stairs with reasonable or no accommodation.
