Overview:
Full time position for Mental Health Case Manager to assist adult clients living with severe mental illness. Position includes working with clients who participate in specialized services for LGBTQ and older adult populations.
Duties:
Provide community-based case management services to individuals suffering from mental illness. Services provided are, but not limited to, symptom monitoring, treatment/service planning, assistance in daily living, supportive services, crisis intervention, linkage/referral/advocacy, and support of client's self-determination. Work in a supportive team environment as well as collaboration with a multidisciplinary clinical treatment team. Meet annual service delivery expectations. Provide transportation of clients in personal vehicle, as necessary. Participate in 24-hour emergency on-call rotation.
Qualifications:
Bachelor's degree preferred. Candidates not possing a bachelor's degree, but with relevant experience will be considered. Transportation and good driving record required as well as personal vehicle insurance for bodily injury/property damage liability. Strong interpersonal skills and ability to provide support in person and by telehealth. Must be able to lift/push/pull up to 20 pounds to support demands within office environment.
