Overview:
Central Connection is a Division of Central Clinic Behavioral Health, and are known as the "front door" for Hamilton County residents who need connection to mental health and/or substance use treatment. Central Connection's primary mission is to provide a standarized entry to a system of cae that ensures that services are available, accessible and of high quality. This role is both community and office based.
This position involves a combination of clinical supervision and direct care responsibilities, specifically for the Central Connection program. Key duties iunclude providing assessments, case management, and clincial supervision for a small team. The program serves clients transitioning to long-term care providers within 90 days, and the ideal candidate will work closely with these individuals, many of whom are frequent users of community services.
Key Responsibilities:
- Supervise and mentor a small team of assessors and case managers, providing guidance and clinical support.
- Provide case management services in the community, including ongoing engagement with clients.
- Conduct client assessments on an as-needed basis, typically referring from hospitals, forensic systems, and the community.
- Maintain a small caseload, typically 8-12 clients, with flexitility to adjust based on need.
- Engage with clients in a way that goes beyond traditional "productivity" metrics, focusing on building strong relationships and providing high-level support.
Minimum Qualifications:
This candidate shoudl be someone who enjoys face-to-face client interactions, is skilled in both clincial supervision and case management, and has a passion for working with individuals who need intensive support and engagement. Experience working with our population is an important factor. Looking for someone with a master's degree in social work, counseling or marriage and family therapy, and independent licensure. Requires reliable transportation, a valid driver's license, and personal vehicle insurance for bodily injury/property damage liability.
